This form is a Civil Subpoena for the personal Appearance and production of any kind of information and things at a court hearing. In other words, this form is used to call the individual to the court hearing. The information provided in the form is used by the individual who receives it to learn about the date, time and address they must appear in the court. The form also contains the Declaration in Support of Civil Subpoena and the Proof of Service of Civil Subpoena.

When is the SUBP-002 form due?
The Civil Subpoena and the Declaration in Support is sent to the witness before the court hearing. The Proof of Service is filed with the court when the Civil Subpoena has been served to the individual.
What information should be provided in the SUBP-002 form?
The Civil Subpoena contains the following information:
- Name and address of the party
- Name and address of the court
- Name of the plaintiff/petitioner
- Name of the defendant/respondent
- Date, time and address of the court hearing
- Date the Subpoena was issued
- Signature of the person who issued the Subpoena
The Declaration in Support of Civil Subpoena contains the following information:
- Name of the plaintiff/petitioner
- Name of the defendant/respondent
- List of the documents to be produced at the hearing
- Reasons for the producing of the above-mentioned documents at the hearing
The Proof of Service contains the following information:
- Name of the person who served the Subpoena
- Address where the Subpoena was served
- Date and time of delivery
The form must also be certified by the corresponding individuals.
What do I do with the form after its completion?
The Civil Subpoena and the Declaration in Support is forwarded to the witness; the Proof of Service is filed with the court.
